The first week

If you are off work for up to 1 week you do not need a medical note from your doctor. If you have been sick for 4 days or more in a row you should 'self certify' using a form (SC2) that you should be able to obtain from your employer. Alternatively you can obtain one from the Benefits Agency.

Unfortunately some employers are insisting on a medical note from the patient's doctor for less than one week's absence from work. If you need a medical note in these circumstances, the doctor will charge a fee for a private certificate. Reception staff will be able to tell you how much this will cost.

Sick Notes for Short Periods of Absence

In the event that your employer requests a sick note for a short period of absence due to illnesses, please CLICK HERE to download the letter to provide to your employer.

After the first week

If you have been off work for more than a week you will need a sick note from your doctor. You can obtain this by contacting reception for an appointment.
