The first week
If you are off work for up to 1 week
you do not need a medical note from
your doctor. If you have been sick
for 4 days or more in a row you
should 'self certify' using a form (SC2) that you
should be able to obtain from your
employer. Alternatively you can
obtain one from the Benefits Agency.
Unfortunately some employers are
insisting on a medical note from the
patient's doctor for less than one week's
absence from work. If you need a
medical note in these
circumstances, the doctor will
charge a fee for a private
certificate. Reception staff will be
able to tell you how much this will
cost.

After the first week
If you have been off work for more
than a week you will need a sick
note from your doctor. You can
obtain this by contacting reception
for an appointment.
