We are open for new
patient registrations. Please
bring your medical card with you to
register. A registration form is
required and you will be asked to
complete a
GMS1 form and
new patient
questionnaire. If you are not on
regular medication you will be asked
to make an appointment for a new
patient check with the Practice
Nurse. If you are on regular
medication you will be asked to make
an appointment with a doctor prior
to issuing your next repeat
prescription.
TEMPORARY
RESIDENTS
You are entitled
to see a doctor anywhere in the
United Kingdom if you are away from
home and in need of medical help.
We are happy to see any people who
move temporarily to our catchment
area. You will be asked to
complete a Temporary Residents form
prior to seeing the doctor or nurse.
